Application forms to set up facility

To set up either or both the facilities the following forms need to be completed and signed by the organisation.

  1. Application for an Employee Benefits Card Facility
    Authorised Signatory/Verifying Officer – Letter of Nomination

  2. Application for a Meal Entertainment Card Facility
    Authorised Signatory/Verifying Officer – Letter of Nomination

The application form for employees (to be completed and signed by both the employee and employer) can be forwarded either by mail or facsimile to PBI or Westpac (02) 9374 7916 – attention Team Leader, Commercial Cards New Business Unit:

  • Employee Benefits Card – Cardholder Request
  • Meal Entertainment Card – Cardholder Request

Salary Sacrifice Agreement

EBC and MEC – Salary Sacrifice Agreements are between the employer and employee and are to be retained by the employer.
