Explanation of the Need for a Direct Debit Request Form

The Meal Entertainment Card (Card) has been designed so that the amount an Employee can charge to the Card is limited to the funds available from salary sacrifice transfers made to an Employee's card account. Generally, overspending will not be authorised.

However, in limited circumstances such as automatic charging of the annual fee or manual processing of charge transactions at merchants that have not required telephone authorisation, the Card account may be overdrawn.

In these circumstances any overspent amount will be charged directly to the employee via an account that they nominate.

Accordingly the Direct Debit Request form provided must be completed by you to authorise this transaction. Completed forms need to be sent to Westpac attached to the Cardholder Request form. No Card requests will be processed without a Direct Debit Request form.

Employees should note that they will be responsible to ensure that their nominated account has sufficient funds to cover any overspent amounts and in particular should take into account that the annual card fee of $160 will be debited to the Card account in the first month the Card is issued.
