Instructions on Cancelling a Card

The Commercial/Business Choice Card Cancellation Closure Request Form is used:

  1. to cancel a card; and

  2. for any unspent money to be refunded by Westpac to the employer facility account nominated at the time the facility was established (any unspent money to be taxed accordingly before being returned to the employee).

Once completed, fax to either PBI on (02) 9344 9400 or Westpac Cards Business Solutions on (02) 9374 7003 (retain the fax confirmation from Westpac for your records).

The employer should collect and destroy the card and provide confirmation the card has been destroyed.

It is also very important that you get the employee to confirm that there are no direct debits established on the card. While such direct debits are against the card terms of use, there is no system block to prevent their establishment. The employee should either confirm in writing that there are no existing direct debits on the card or provide cancellations for any existing direct debits that have been established contrary to the conditions of use.
