Application forms to set up facility
To set up either or both the facilities the following forms need
to be completed and signed by the organisation.
- Application for an Employee Benefits Card Facility
Authorised Signatory/Verifying Officer – Letter of Nomination
- Application for a Meal Entertainment Card Facility
Authorised Signatory/Verifying Officer – Letter of Nomination
Once
completed, the forms need to be forwarded either by mail or facsimile
